REQUEST FOR RECONSIDERATION

This request will be reviewed by the Library Director, who will respond in writing within thirty days of receiving this form. You may appeal the decision of the Library Director to the Library Board of Trustees, in writing, within thirty days of the director’s notification. The Library Board will respond within thirty days of the appeal date.
